Please read and fill in all the blanks to apply for any volunteer or coaching positions with Port Colborne Minor Hockey.
  1. COACHING & VOLUNTEER APPLICATION

    REP PROGRAM

    PCMHA COACHING EXPECTATIONS

    We are looking for coaches and volunteers who are committed to PCMHA, and want to work with us to provide the best possible hockey programming for the players of PCMHA.

    1.    Familiarize myself with, and abide by, the POLICIES OF THE PORT COLBORNE MINOR HOCKEY ASSOCIATION (PCMHA)

    2.    Upgrade my credentials as needed by the PCMHA and attend all Coaches meetings scheduled by PCMHA;

    3.    Comply with the Manuals of Operations consisting of the PCMHA Constitution and the NDHL Constitution

            along with Appendix “A”,”B1”, & “B2”

    4.    Comply with the MANUALS OF OPERATIONS OF THE ONTARIO MINOR HOCKEY ASSOCIATION (OMHA).

    5.    I have read and understand the PCMHA CODE OF CONDUCT and the OMHA CODE OF CONDUCT

    6.    I understand that any proposed Bench Staff must have the proper and up to date TEAM OFFICIAL QUALIFICATION REQUIREMENTS set out by the OMHA and Hockey Canada.    

    7.    I understand that any proposed Bench Staff will also be required to complete an online application, and any person put forward as a potential Bench Staff member will be subject to PCMHA Board approval.

     

  2. ALL REP COACH APPLICATIONS ARE DUE BY MARCH 2nd, 2019 at 11:59 PM.

    Interviews will be start shortly after this date.  Interviews will be conducted by the Coach Selection Committee. Following the interview(s), the Coach Selection Committee will make their recommendation for the best overall candidate at or just before the Annual General Meeting. You will be notified either way on the decision made.

    SPECIAL REQUIREMENT TO ALL PCMHA REP COACHES

    All coaches applying with the PCMHA REP Program are required to submit a complete SEASONAL PLAN at their interview That would include: 
    -  Skill & drill examples
    -  Tournaments 
    -  Fundraising 
    -  Off ice training 
    -  Coaching philosophy with respect to players/parents and staff members.

  3. I understand that completing a coaching application for the PCMHA does not ultimately guarantee me a coaching position with the PCMHA
  4. I authorize PCMHA to collect personal information appropriate to the information applied by verifying character references supplied.

POLICE VULNERABLE SECTOR CHECKS (PVSC)

    Maximum # Files: 1. Maximum File Size: 4MB.
  1. I understand that all volunteers must have a Vulnerable Police Sector Check (VPSC), on file with PCMHA, and if not, to provide proof of an updated VPSC to PCMHA.  

    Please NOTE: A Vulnerable Police Sector Checks (VPSC) is also known as Police Background Checks and is valid for only 3 years.

    If you have a valid VPSC on file with PCMHA, you would be required to sign the Declaration of Offense document for each season PRIOR to the season of issue.  This includes ANY Head Coach or proposed Bench Staff volunteer, or any on-ice volunteer.

    Once you have your Police Vulnerable Sector Check receipt, please scan and include this with your coaches application.  Once you receive the actual embossed sealed PVSC paper, please make arrangement to give it to 1st Vice President, Helecia Gross.
