  1. INFORMATION SHEET FOR HOUSE LEAGUE & LOCAL LEAGUE
    COACHES, TRAINERS AND VOLUNTEER POSITIONS

     

    Please read the following information below to have an understanding of what is expected from all PCMHA volunteers.

    As a PCMHA Volunteer, I will...

    1.  Familiarize myself with, and abide by, the policies of the Port Colborne Minor Hockey Association (PCMHA);
    2.  Upgrade my credentials as needed for the volunteer position within PCMHA;
    3.  Attend all Coaches meetings scheduled by the PCMHA;
    4.  Comply with the Constitutions, Code of Ethics, and Fair Play Codes of the Port Colborne Minor Hockey Association (PCMHA),the
    Ontario Minor Hockey Association (OMHA), & the Niagara District Local League (NDLL) documents.
    5.  Read the Code of Conduct and Coaching Job Descriptions that are posted on the websites www.portminorhockey.com & www.omha.net
    6.  I understand that any proposed Bench Staff will also be required to complete an online Information Sheet..
    7.  I understand that all volunteers will be subject to a Vulnerable Police Sector Check.  If you have a Vulnerable Police Sector Check that is less than three(3) years old, you will be required to sign a Declaration of Offense document.  If your Vulnerable Police Sector Check is more than three(3) years old, you will be required to obtain a new one.

  2. SPECIAL REQUIREMENT TO ALL PCMHA HOUSE LEAGUE AND LOCAL LEAGUE VOLUNTEERS

    All volunteers applying for any position with PCMHA are required to have a Vulnerable Sector Background Clearance Certificate (Police Background Check)

    All  Coaches, Trainers and other Volunteers are required to submit this Information Sheet A.S.A.P. by pushing the "Submit Form" buttom at the bottom of the page.

    In order for  this information sheet to be submitted, you must reply to all the questions within this Information Sheet.

VULNERABLE SECTOR BACKGROUND POLICE RECORD CHECK
  1. Allowed extensions: .jpeg, .jpg, .png, gif, .pdf, .doc, .docx.

CRIMINAL OFFENSE DECLARATION
If you have submitted a Vulnerable Sector Background Police Record Check to PCMHA within the last 3 years, please answer Yes or No on this Declaration Page, If nothing has changed, we will verify your information with our records.
  1. VULNERABLE SECTOR BACKGROUND (POLICE CHECK) – (VSBC)

    As a requirement by PCMHA, the OMHA, and Hockey Canada, all volunteers are required to have a valid VSBC on file with PCMHA.  All VSBC’s issued have an issue date on them; if your issue date is older than three (3) years from the date of issue, you will need to renew your VSBC in order to up to date.

    When you know that you have been selected as a coach or other volunteer, please submit your Police Check ASAP.

    If you already have a current CSBC that is not more than 3 years old from the issue date, you can include it as an attachment within this application.

    If you DO NOT have a valid CSBC, you must obtain a new updated one ASAP.

    In order to obtain or update a VSBC, you MUST first print off a Vulnerable Sector Police Record Check Confirmation Form which is found under the Resources tab on the top tool bar on the website.  Then your best option would be to bring that page to the Port Colborne Police Detachment on the corner of Fielden Ave. and Killaly St. W. and see Kim Gravelle and maybe request 4 or 5 embossed copies.

    Kim will give you a receipt when you apply and pay for your VSBC.  You should then submit that receipt to Randy Huggins (House League Convener) so that PCMHA knows that you have applied.  Once you get the actual VSBC through the mail, you once again must submit the embossed original copy to Randy Huggins.  He will submit your VSBC to PCMHA and your information will be uploaded to Hockey Canada.

Coaches of House League/Local League/ Minor Development / Rostered Select in divisions below Novice (Initiation & Tyke) must hold Coach 1-Intro to Coach qualification - No other qualifications are accepted 2. Coach 2-Coach Level ‘Trained’ or higher: Coach 2-Coach Level ‘Trained’ or ‘Certified’; Development 1 ‘Trained’ or ‘Certified’; High Performance 1 & 2 ‘Trained’ or ‘Certified’ 3.
